Pay: £12.35 per hour

Hours: Full time, Monday to Friday, 9:00am until 5:00pm

Office Angels are currently recruiting for a Patient Care Coordinator to work for a large hospital group trust located in the Dudley area. This is a pivotal role to the organisation which will be aligned to the Cancer Services multidisciplinary team (MDT). Multidisciplinary teams (MDTs) are the mechanism for organising and coordinating health and care services to meet the needs of individuals with complex care needs. The teams bring together the expertise and skills of different professionals to assess, plan and manage care jointly.
The co-ordination of a patient journey is a critical factor in ensuring the timely and effective delivery of care. This role provides a means of improving quality of care for individual patients by ensuring the prospective tracking of their journey and involvement of the specialist Multidisciplinary Team. As the Patient Care Co-ordinator; you will work as part of an existing team and hold the responsibility to assist with the day-to-day provision and delivery of cancer services through the application of effective administration and data management. This is a complex role that requires a pro-active approach, excellent organisation skills and trustworthiness with confidential information.
Role responsibilities - you will be comfortable and excel in the below:
Providing clerical and administrative support to the specialist Multidisciplinary Team (MDT)
Identifying patients for discussion prior to the MDT meeting
Arranging meetings, preparing agenda's for meetings that take place each week to discuss patients cancer pathways
Preparing & recording data and care outcomes on clinical systems
Preparing agendas for the weekly MDT meetings
Tracking the progress of cancer patients through their journey
Completing electronic records, ensuring timely appointments
Collection & recording of specific clinical data
Experience / Skills required for this role:
Previous Administration / Co-ordination experience within a health care environment (Desirable)
Ability to work within a team alongside a range of health care professionals
Excellent organisational skills to support the planning and flow of patients through treatment
Pro-active approach to patient care and team support
Experience of administration tasks within a busy patient focused service
Ability to multi-task and cover all aspects of administration
Ability to manage a varied workload and work well under pressure
Ability to work to deadlines and complete tasks fully
Caring and empathetic attitude
Computer literate and ability to integrate computer based information across platforms
